Our community has multiple care levels of service for residents who live in the Assisted Living neighborhood. The care level is determined by an in-depth, comprehensive assessment completed by a community resident care leader. A number of factors are considered when completing the assessment, including physical abilities, cognitive function, risk factors for conditions that require staff interventions, complex medication regimens, coordination of care with outside providers, and several other factors.
Each resident’s abilities and needs are different, and based on this assessment, a personalized service plan is developed to reflect the support needed for the individual. Care levels and associated costs are based on the combination of these unique contributing factors.
